The following items need to be brought to the school office:
- Birth Certificate: The original birth certificate or other reliable proof of a student’s identity and age together with an affidavit (statement made under oath and notarized) explaining why the original birth certificate cannot be provided.
- Immunization Records or Exemption Certificate: Immunization information can be found on the Student Services Website. If you would like to exempt your student you can complete the Online Immunization Education Module and bring a copy of the exemption certificate to the school office.
- Proof of Residence: Must be in the parent/guardian name and show the current address. Examples could include one of the following; mortgage statement, rental agreement, utility bill, internet bill, building permit, etc.
- Legal Documents: Any legal documents outlining custody or student contact restrictions.
